This is the case of 33 yrs old lady who was diagnosed to have ‘AIDS’. The patient was unaware of her disease.

Initially she was suffering from generalized fever with chills. She was asked to undergo HIV test, as her husband was diagnosed +ve for AIDS earlier. So, she was investigated and was found to have very low CD4 counts. Ultimately she was diagnosed to be suffering from AIDS.

Suddenly small pustules started appearing on her extremities, which started spreading over the entire body. There were discharges oozing from the boils which had foul odor and she failed to respond to any treatment. She was shifted from Dhule Hospital to a multispecialty Hospital in Pune.

Day by day her condition worsened. She got completely bed ridden. The offensiveness of discharges increased, giving the patient a dirty look, like that of a burn patient. The offensiveness was so marked that the nurse’s avoided dressing the boils. She was asked to be taken home from the hospital, as her condition was getting worse day by day and there were no hopes left.

Somehow, the relatives came to know about my hospital and they came to visit my hospital without bringing the patient.

The relatives brought the patient’s photograph and a short video recording of her condition. Detailed case history of the patient was taken by communicating with her on the phone and by the information provided by her relatives. When I saw the video recording, I got terrified as the patient was completely wrapped in bandages all over the body. Even in the video recording she was looking so ugly that I could not even imagine the intensity of pain patient must be going through.

HIV : Acquired Immunodeficiency Syndrome (AIDS) is caused by the Human Immunodeficiency Virus (HIV). The initial response to HIV invasion is a modest decline in the number of circulating T4 cells. Infected people experience a brief flu-like illness, with chills and fever, but the immune system fights back by making antibodies against HIV and the number of circulating T4 cells recovers nearly to normal. Although infected people test positive for HIV antibodies, they typically have few clinical signs or symptoms and do not yet have AIDS.

Some weeks after infection with HIV, the host develops antibodies against several proteins in the virus. Usually, antibodies appear in blood within 6 weeks to 6 months after exposure.

Over the next 2-10 years, the virus slowly destroys the T4 cell population in lymphatic tissues throughout the body. As immune responses weaken, people develop certain indicator diseases (diseases that are rare in the general population but common in AIDS patients). At this point, the diagnosis of AIDS is made.

A simplified definition of AIDS includes anyone infected with HIV and having a T4 lymphocyte count under 200/mm3 of blood. (Normally, the T4 count would be about 1200/mm3).

Management of HIV in modern science is very costly ranging from 2000 to 10000/month. This relates to only antiretroviral drugs, which are helpful in keeping CD4 count in the body. Above 200, there are other drugs also that the patient has to take related to infections present in the body. The patient has to take the entire retroviral drugs without missing a single dose until the end of his life. These drugs have many side effects.

12.06.2008 :
Test for HIV Antibodies (I & II) : The test is +ve for antibodies against HIV-I.

13.06.2008 :
CD4 Count : 285 cells / cumm
CD8 Count : 1234 cells / cumm
Total CD3 Average: 1517 cells /cumm

14.06.2008 :
Western Blot is positive for HIV-I and negative for HIV-II.

22.07.2008 :
CD4 Count : 249 cells / cumm
CD8 Count : 909 cells / cumm
Total CD3 average : 1192 cells / cumm
CD4/CD8 ratio : 0.27

Her complaints in detail were as follows :

C / C :

Pustules all over body since 10 days.
Location : All over body except scalp & palms of hand. Lt. to Rt.
Sensation of severe burning.
Character : Initially small in size; suddenly becomes big in size with watery discharge.
Painful on pressure.
Discharge : Profuse, offensive.
Small pustules get burst by itself.

My observation of photograph : Scab formation over pustules with underlying pus.

Chills all over body since morning.
< Slight movement. > Dark room.
< Touch.
< Light.
< Tight bandage.
Associated with watering from eyes with burning.

Dry cough since 2 - 3 days.

Difficulty in speaking.

Past History :
H/o Fall on head 3-4 yrs back.

  • Appetite : Good. Can’t tolerate hunger.
  • Desire : Cold drinks++ / Sweets++ / Pungent+ / Sour / Green Veg. / Milk / Milk Products / Fruits / Fruit Juices/ Oily.
  • Food< : Fatty food< Acidity.
  • Head : Headache (occ.) Frontal region.
    • < Change of weather. > Tight bandage.
    • Hair fall since 1 yr.
  • Eyes : Watering from eyes with burning.
    - Discharge – Profuse & sticky.
    - Lt. to Rt.

  • Throat : Sour throat occ.

  • Thirst : Thirsty. S.Q.S.I.
  • Teeth : Caries of teeth.
  • Gums : Bleeding gums while brushing.
  • Chest : Pain in chest occ.

  • Bowel : Stool hard, dry, and require straining.
    - C/o Piles since 9 yrs, external bleeding piles.

    - Burning & severe stitching type of pain at anal region after stool.

      - Profound weakness after stool
  • Skin : Itching on both hands & legs.
    < Night > Scratching after. - Sometimes bleeding due to scratching.

 
  • Female : Menarche : at the age of 14 yrs.
    Regular cycle, Previously 6-7/28 days.
    Now 3-4/28 days since 5-6 mths.
    Dark red, blackish.
     
  • Mind :

    Tell me about your childhood ?
    I had 2 brothers & 1 sister, myself being the eldest. I was a responsible child as mother did not keep well, most of the times. I had to look after my brothers & sister and had to start working very early along with studies.

    I was calm and did not get angry easily.

    How is your nature now ?
    Even now, I don’t get angry very easily. I keep calm. If I get angry, I express it by talking and otherwise I keep it in my mind.

    I don’t talk much. I always desire company and mix up easily with everybody at home. I stay in a joint family and although I am the younger daughter in law, the responsibility of the entire family is on me.

    I like to work, look after everybody, and care for everybody at home.
    I am a religious person and have great faith in God and hence I am very hopeful about my recovery.
    Her relatives added that she has a very cheerful nature and also enjoys jesting.

    What are your worries?
    At present, I am very much worried about my health and want to recover as soon as possible. From last 6 months, I have been constantly worrying about my husband’s health since he is not responding to any treatment.

    What do you fear about ?
    I fear darkness, thunderstorm, water, and heights.

    How is your relationship with other family members?
    Generally, I share good relationship with everybody at home, only there are occasional conflicts with the elder sister-in-law. Relationship with husband is good and as such no big conflicts since he is also very understanding.

    Response :
    The doctors in super specialty hospital were constantly insisting the relatives to take her home, but after giving her the homoeopathic medicine, I just asked them to stay for two more days.
