Acute Renal Failure (A.R.F.)

Acute Renal Failure (ARF), also called acute kidney failure, is a quick loss of renal function due to damage occurring to the kidneys. It leads in the retention of nitrogenous (urea and creatinine) and non-nitrogenous waste products that are generally excreted by the kidney. Depending on the sternness and duration of the renal dysfunction, the accumulation is accompanied by metabolic disturbances, such as metabolic acidosis (acidification of the blood) and hyperkalaemia (elevated potassium levels), changes in body fluid balance and effects on several other organs. It can be characterized by oliguria or anuria (decrease or end of urine production), although nonoliguric ARF might occur. It's quite a serious disease and treated as a medical emergency. Acute or abrupt kidney failure is the sudden loss of the ability of the kidneys to remove waste and concentrate urine without losing electrolytes.

Causes of Acute Renal Failure
  • Anorexia, breath odor, bloody stools, brusing easily
  • Changes in Mental Statusor mood, decreased appetite, decreased sensation, particularly in the hands or feet, dyspnoea
  • Fatigue, flank pain (between ribs and hips), haematuria, hand tremor, headache, high blood pressure, lethargy
  • Metallic taste in mouth, nausea or vomiting, could last for days, palpitation, nosebleeds, constant hiccups, seizures, slow, sluggish movements, swelling of the ankle, feet and leg
  • Changes in Urination
    • Decrease in the amount of urine
    • Excessive urination at night
    • Stopping of urination completely
General Management of Acute Renal Failure
  • Determining the appropriate cause, maintaining fluid, electrolyte balance
  • Limiting fluid to an amount equal to the volume of urine produced, intake of salt should be restricted, protein, sodium and potassium intake should also be restricted
  • Diet needs to be high in carbohydrate content, appropriate anemia and maintaining daily record of body weight, fluid intake, urine output, blood urea and serum electrolytes
  • Dialysis is not essential in all the cases. It's recommended and life saving especially when serum potassium is extremely high, anuria, deranged mental status, precarditis etc.
